Albemarle Regional Health Services (ARHS) and the hospitals in the Albemarle region of care including Vidant Bertie, Vidant Chowan, Vidant Roanoke-Chowan, and Sentara Albemarle Medical Center are working together to complete the comprehensive 2022 Community Health Needs Assessment. ARHS and the hospitals would value your input as well as the opinions of others in your network and community.

This assessment is being undertaken by a partnership of 33 counties across Eastern North Carolina. It allows partners to better understand the health status and needs of the community and use the knowledge gained to implement programs that will benefit the region.

This survey allows community members like you to tell us about what you feel are important issues for your community. We estimate that it will take about 10 minutes to complete the question survey. Your answers to these questions will be kept confidential and anonymous. Please complete the survey by May 15, 2021.

Please share this link broadly, with anyone you know including employees, civic organizations, clubs, faith-based or community groups! Participation from our community members in Pasquotank, Perquimans, Camden, Chowan, Currituck, Bertie, Gates, and Hertford counties will be invaluable.

Once we have gathered all of the surveys, we plan to compile this information and use it to develop a community health improvement plan with our community public health partners.
